Carolina Psychological Health Services is accepting new clients and we look forward to being a resource for you and your family.

New Client Information

Becoming a client with us is simple.

To schedule an appointment, call our office and let our receptionist know you would like to become a new client. For minors, the caller must be the client’s biological parent or legal guardian (must provide legal documentation, for example, a court order or Power of Attorney). 

Please be prepared to provide your full name, date of birth, address, phone number, email address and insurance information, including policyholder name, date of birth, social security number, and address. For the minor client: the biological parent or legal guardian (with legal documentation) must be present at the initial appointment. After you provide the initial information, you will receive an email which contains a link to set up your patient portal and seamlessly complete the new client forms.

Each scheduled appointment is reserved exclusively for you. If you cannot keep your appointment, please call and let us know. We require at least a 24-business-hour notice to cancel or reschedule any appointment.

What to Expect At Your
First Appointment?

Please arrive 30 minutes before your first appointment to allow time to go over the required paperwork. For subsequent appointments, we ask that you arrive approximately 15 minutes before your appointment to complete the check-in process. Your first appointment is critical as we get to know one another on a deeper level to address your needs better. You need to bring your health insurance card(s), a list of current medications (and dosage), and a non-expired government photo identification (driver’s license, military I.D. card), and form of payment. We accept cash, check, money orders, Discover, Mastercard and Visa.

For tele-health visits, please ensure that you have the correct link to join your session. You can also access your session directly through the patient portal. 

Due to COVID-19, our office is currently enforcing additional precautions to protect the health and safety of our clients, staff and loved ones. Please be sure to wear an appropriate face mask to cover your mouth and nose when entering the building regardless of vaccination status. If you do not have one, please inform the receptionist as you check in and one will be provided for you.

Important Notice:

While we are considered  in-network for several insurance companies, prior to making an appointment, please check your eligibility with your insurance company. Our staff can assist you with questions about your insurance and will be happy to help you determine your estimated cost of treatment
